4. Fotocopy SITU sebanyak 3 lembar
5. Pas Foto warna (3X4) sebanyak 2 lembar
6. Materai 6000 sebanyak 3 lembar
7. Map Plastik biru sebanyak 2 buah
8. Mengisi formulir SIUP sebanyak 3 rangkap

Biaya SIUP:

• SIUP Kecil (kekayaan bersih) diatas 5.000.000 = Rp.100.000
• SIUP kecil (kekayaan bersih) dibawah 5.000.000 = Rp. 50.000
• SIUP Menengah = Rp. 250.000
• SIUP Besar =Rp. 500.000

Biaya Pendaftaran ulang SIUP (Her Registrasi) :

• SIUP Kecil = Rp. 20.000
• SIUP Menengah = Rp. 30.000
• SIUP Besar = Rp. 50.000
• Legis SIUP Kantor Cabang Pusat / Perwakilan pusat = Rp. 75.000
• Legis SIUP lokal = Rp. 20.000.
